Clinic Visits are by appointment only. Special appointments are scheduled only for extreme circumstances and are not always available. Please call our office at 952.285.1752, if you need to cancel or reschedule.

We allow patients a half-hour grace period for late check-in, after which time we require rescheduling. Priority is given to patients arriving promptly for their appointment.

Fees

First Visit $50
Includes medical history and physical exam, consultations with a Registered Dietitian and a physician, and if medically qualified, placement on phentermine for a one week period.

Monthly (4 week) Follow-up Visit $90
For patients receiving phentermine. Includes follow-up behavioral consultation and evaluation by a physician, with adjustments in diet, physical activity and phentermine dosage as indicated, and continuation on phentermine for a four week period.

As a convenience to our patients, we dispense the medication, phentermine, to our patients at each office visit, in a maximum of four-week allotments.

Cost of medication and all instruction materials are included in the above fees.

Payment is at time of visit with no advance fees or contracts.

We currently accept cash and personal checks, and will soon be accepting credit cards.

Upon request, we will provide information for patients seeking insurance reimbursement. However, to keep our fees affordable, we do not deal directly with third-party payers, due to the extensive labor costs involved.
